1. I understand that prior to presenting myself on NYCBC property, I have self-assessed using Toronto Public Health’s screening tool and do not feel unwell. If I am unwell, I am not to come to NYCBC.

2. I will pre-register for a particular service or meeting and will not present myself at NYCBC without prior confirmation from NYCBC. I understand that I will be turned away if I come to NYCBC without prior arrangement. I understand that the information collected, as per Toronto Public Health guidelines, is for purposes of health screening and contact tracing, and will be deleted after 30 days.

3. I understand that I will self-screen upon entry into the building, and will answer all questions honestly.

4. I understand that NYCBC will designate a health and safety coordinator for each meeting. In the event I begin to feel unwell while at NYCBC, I will return home immediately and self-isolate. If assistance is required, I may contact the safety coordinator on site.

5. I understand that a face covering is mandatory, and that my face will be appropriately covered. The only exception is for those who are under 2 years of age. I understand that proper mask hygiene includes having the mask or face covering cover my nose, mouth and chin securely without gaping.

7. I will practice proper physical distancing, and hand and respiratory hygiene on NYCBC premises.

8. I understand that physical distancing measures such as designated directions or paths of travel, limited seating capacities, restrictions to different areas are in place, and that I must abide by them.

9. I will proceed directly to the space/seat that I am provided by NYCBC, and will not detour to other places. I will use the designated path of travel to get there.

10. I understand that as the COVID-19 situation constantly evolves, NYCBC policies and procedures will adapt in parallel. It is my responsibility to understand current policies and procedures to the best of my ability.

11. I agree to hold harmless and indemnify NYCBC, its staff, directors, members and visitors against any and all claims and actions as a result of entering NYCBC’s premises, following protocol outlined by NYCBC, or using hygiene products provided by NYCBC.
