NYCBC Softball On-Field Waiver:
By signing my name below, I agree to abide by the rules and requirements as set out by North York Chinese Baptist Church (NYCBC) and to play the game of slo-pitch softball in the attitude and spirit befitting of a participant in accordance with the guidelines set out by NYCBC.

I consent to the collection, use and disclosure of my personal information as outlined here: I acknowledge that NYCBC may obtain photographs or videos of me in connection with my participation in the NYCBC Family Softball Tournament. I grant NYCBC permission to display, publish, distribute, use, and/or print these photographs or videos in connection with NYCBC sanctioned promotional materials, including but not limited to: announcement inserts in bulletins or media on the NYCBC website. I also consent to the collection, use and disclosure of my personal information as required by The City of Markham -Facility Rentals COVID-19 Acknowledgement, Waiver and Indemnification form.

I release from any and all liability NYCBC, its staff, advisors, board of deacons, NYCBC staff affiliates, tournament organizers, coaches, umpires and volunteers from any and all claims, demands, losses, costs or damages, whether arising from negligence or otherwise arising from my or my child’s participation in the NYCBC Family Softball Tournament and in any and all related activities sponsored in whole or in part by NYCBC. I acknowledge and accept full responsibility for my own actions and consequences as a result of my participation in the NYCBC Family Softball Tournament. I understand that under no circumstances shall NYCBC and its staff, advisors, board of deacons, NYCBC staff affiliates, tournament organizers, coaches, umpires, and volunteers be held responsible for any injuries suffered resulting from my own participation in the NYCBC Softball Tournament. If I am under 18 years old at this time, by clicking below, my parent/legal guardian is aware of my participation in this event and agrees to the content of the above On-Field Waiver.

COVID-19 Waiver
The World Health Organization has declared COVID-19 a worldwide pandemic. COVID-19 is an extremely contagious virus that spreads easily through person-to-person contact. Provincial and Federal authorities recommend physical distancing and mask use to prevent the spread of COVID-19. Contracting COVID-19 can lead to severe illness, personal injury, permanent disability, and death. Older adults and people of any age, including children, who have serious underlying medical conditions might be at higher risk for severe illness or death from COVID-19. Use of City of Markham facilities and participating in the NYCBC Family Softball Tournament could increase the risk of you or your child contracting COVID-19. The City of Markham and NYCBC have put in place preventative measures to reduce the spread of COVID-19, however, the City of Markham and NYCBC cannot guarantee that you or your child will be safe from illnesses such as COVID-19, or colds and flu while using the City of Markham facilities or participating in this Tournament. By participating in the NYCBC Family Softball Tournament, you and your child are exposed to the risk of contracting or spreading COVID-19.
I understand that my/my child’s participation in the NYCBC Family Softball Tournament is voluntary and is not required. By clicking below to acknowledge acceptance and agreement of this waiver, I acknowledge that I have carefully read the above, and understand the risks of COVID-19 associated with participating in the NYCBC Family Softball Tournament. I voluntarily assume such risks, including the risk of serious illness, debilitating injury, or death to me/my child. I further acknowledge that I understand that the risk of exposure to, contracting, or spreading COVID-19 may result from the acts, omissions, or negligence of myself and others, including but not limited to the Tournament organizers, volunteers, other participants, and their families. I knowingly assume such risks, including the risk of serious illness, debilitating injury, or death to my child/myself. To reduce risks of contracting or spreading COVID-19, I agree to comply with any safety protocols put in place by NYCBC and The City of Markham during my participation in the NYCBC Softball Tournament.

By clicking below to acknowledge acceptance and agreement of this waiver, and in consideration for providing me/my child the opportunity to participate in the NYCBC Family Softball Tournament, I, along with next of kin, heirs, executors, administrators and assigns of myself or my child, voluntarily agree to waive and discharge any and all claims against NYCBC and The City of Markham related to or arising out of COVID-19, and voluntarily release NYCBC and The City of Markham from liability for any exposure to or illness or injury from COVID-19, including claims for negligent actions of NYCBC, The City of Markham, or its officers, directors, organizers, volunteers, related to or arising out of COVID-19, on behalf of myself and/or my child to the fullest extent allowed by the law. By clicking below to acknowledge acceptance and agreement of this waiver, I agree to release, discharge, and hold harmless NYCBC and The City of Markham, or its officers, directors, organizers, volunteers, from all liability, claims, causes of action, or demands, including attorney fees, fines, fees, or other costs (e.g. medical costs) arising out of any exposure to or illness or injury from COVID-19.
