I hereby authorize North York (Chinese) Baptist Church to use the information collected in this form to be used for registering my car license plate in the church’s parking system and for internal purposes only, according to the `Personal Information Protection and Electronic Documents Act’.  I understand and agree that this parking privilege only applies when I participate in Church activities. Otherwise, I agree to pay when conducting private businesses while parking at church.
為了符合加拿大「保謢個人資料及電子文件法案」。我授權北約(華人)浸信會使用以下所填寫的資料作為教會泊車登記系統記錄所用。此資料不會用於教會以外。我理解並同意，此泊車優惠只適用于參加教會活動，若是進行私人事務時，我願意付費泊車。

Release and Waiver Declaration 放棄及豁免聲明
In consideration of the benefits derived from my participation in Church Free Parking Application and other good consideration. I hereby release and forever discharge NYCBC, its directors, employees, volunteers and agents from and against any and/or all liabilities, responsibilities, claims, actions, causes of actions, and demands which may arise directly or indirectly from my participation in the free parking, and hereby waive all my rights to claim and/or take any and/all kinds of action against NYCBC, its directors, employees, agents and volunteers. The above Release and Waiver shall enure to the benefit of NYCBC, its directors, employees, agents and volunteers, and shall be binding upon the undersigned and the undersigned’s spouse, family members, heirs, successors, executors and assigns.
考慮到本人參與北約(華人)浸信會申請教會免費泊車登記的優惠，特此表明本人放棄並永遠免除對北約華人浸信會，其董事，僱員，義工及代理人，由於本人參與免費泊車，直接或間接引發的後果，作出追究，問責，索償，行動，行動原因及要求。特此豁免我等一切索償，及/或任何及/或各種針對北約(華人)浸信會，其董事，僱員，代理人及義工採取行動之權利。以上放棄及豁免，將確保北約(華人)浸信會，其董事，僱員，代理人和義工之權益。以下簽署人及配偶，家人，子嗣，繼承人，執行人及受指派人士將共同信守。(條文詮釋以英文版本為準)
